    Quality Comprehensive Healthcare

    Crossroad provides primary healthcare to residents of inner-city Cincinnati at a quality level that could be found in any suburban physician office. Services include general medical care for all ages, gynecology and initial prenatal care, counseling and podiatry. Bi-lingual Spanish healthcare professionals available. Emphasis is placed on educating patients about the value of leading healthier lives. Most commercial insurances accepted as well as Medicaid, Medicare and Self-pay at a sliding fee scale.

    Since 1992, the Directors and staff of Crossroad Health Center have been quietly touching the lives of thousands of people living within inner-city Cincinnati. Founders of Crossroad decided to open the Center after discovering that many underserved Cincinnatians were not getting the level of healthcare that others in more affluent communities were receiving. These same healthcare professionals showed a dramatic level of commitment to their cause by living in the neighborhood they were going to be delivering healthcare services. They chose Over-the-Rhine, one of Cincinnati’s most socio-economically challenged neighborhoods. This focus on reducing health disparities has been a priority for Crossroad - long before the recent focus on “health disparity reduction” became popular.

    2010 Service Highlights

    • Provided 18,279 visits for 6554 patients, a 30% increase from 2009.
    • 51% of patients are African American, 30% are Caucasian and 12% are Hispanic/Latino.
    • 90% of patients have incomes at or below 200% of the Federal Poverty Level (annual income of $44,100 or less for a family of four).
    • 23% of patients are uninsured; 50% of adult patients are uninsured.
    • 20% of adult patients have a diagnosable mental illness.
    • Provided 357 visits for mental health counseling.
    • 3883 patients are children.
    • 82% of children at 24 months of age were fully immunized. The state average for community health centers caring for the poor is 65%.
    • Participation in WeTHRIVE! and Healthy Kids Ohio & The Ounce of Prevention Project - two childhood obesity collaboratives that help Crossroad prevent, identify, evaluate and treat childhood overweight and obesity. Patients ages two and older are screened for weight issues and parents receive counseling and education on age appropriate nutrition and physical activity to treat and prevent obesity. Children who are overweight or obese are linked to community resources that are affordable, accessible, safe and effective.
    • Crossroad Pediatrics West grew quickly in 2010, serving more than 1400 patients thus reaching capacity. Crossroad is seeking a larger office space and hiring another pediatrician to allow for further growth to address the high demand for accessible and affordable health care for children on the West side.
    • Crossroad Pediatrics West immunization rate increased from 29% at the opening of the office to 85% a year later.
    • Average blood sugar control in patients with diabetes was better than the state average for community health centers.
    • 68% PAP smear rate for female patients - well above the 39% state average for community health centers serving patients with low income low.
    • Began implementing Electronic Medical Records (EMR) in 2010 in order to continue to provide best practice medicine to patients. EMR will be fully functional in 2011.
    • Entered into the Beacon Project, a two year process to become a nationally certified "Patient Center Medical Home". Currently, there are only 20 medical practices in the greater Cincinnati area which have this national certification. The two year program will help Crossroad improve many aspects of patient care.
    • Annual patient satisfaction survey showed Crossroad to meet or exceed regional averages in 20 out of 25 categories including: staff are friendly and helpful and answer questions; providers listen, spend enough time with patient, and give good advice and treatment. 96% of patients would recommend Crossroad to their family and friends.

    When you register online for the Climb for Crossroad, Crossroad only collects the information you submit to us. This information is only used for the purpose of registering you for the Climb and is never shared outside the organization. The type of information collected can include: Name, Address, E-mail address, and Telephone number. This information is added to our donor list in order to send acknowledgements of your donations and for future mailings such as newsletters and our Annual Report. To prevent unauthorized access, maintain data accuracy and ensure the correct use of information, we have put in place appropriate physical, electronic and managerial procedures to safeguard and secure the information we collect from you. At any time you may request not to receive any mailings from Crossroad Health Center by informing us by e-mail, telephone, in writing or in person. We retain your request so that it will be respected in the future.
