Care Coordination Team

Work closely with a team of graduate students and young professionals committed to closing the gap between clinical management and self management for Crossroad patients with chronic conditions such as diabetes, hypertension, and asthma! Team members collaborate with patients, families, community partners, health systems, and health plans to coordinate and track care, ensuring that patients have improved care experience, better health outcomes, and decreased health expenditures
For Current Students

Commitment

30 hour/week (although we welcome 40 hour/week) for one semester (10-14 weeks) or
15 hour/week for two semesters. 

Requirements 

We require one of the following:
  • Bachelor's degree
  • Completion of the Crossroad Volunteer Internship Program
  • Significant commitment to the care of vulnerable populations

Future anthropologists, epidemiologists, social workers, nurses, and physicians are encouraged to apply. We currently have participants from the University of Cincinnati, Xavier University, Northern Kentucky University, Miami University, Mount St. Joseph University, post-baccalaureate students, and other young professionals looking to enter the field of healthcare.

Application

We recruit new Care Coordinators throughout the year. Application to the Care Coordination Team is a competitive process with two basic components: 

1) Online Application
  • Professional Resume (2 page maximum)
  • Supplemental Questionnaire (applicants will be contacted with a link)
2) Interview with Program Coordinator and Current Team

Interested students should submit their Professional Resume to crossroadhc@gmail.com to begin the application process. Eligible applicants will be contacted with the Supplemental Questionnaire and invited to interview on a rolling basis.
