Care Coordination TeamWork closely with a team of graduate students and young professionals committed to closing the gap between clinical management and self management for Crossroad patients with chronic conditions such as diabetes, hypertension, and asthma! Team members collaborate with patients, families, community partners, health systems, and health plans to coordinate and track care, ensuring that patients have improved care experience, better health outcomes, and decreased health expenditures

Commitment

30 hour/week (although we welcome 40 hour/week) for one semester (10-14 weeks) or
15 hour/week for two semesters. |

Future anthropologists, epidemiologists, social workers, nurses, and physicians are encouraged to apply. We currently have participants from the University of Cincinnati, Xavier University, Northern Kentucky University, Miami University, Mount St. Joseph University, post-baccalaureate students, and other young professionals looking to enter the field of healthcare. |
