Reports to Chief Operating Officer


Responsibilities:
  • Organize and schedule meetings and appointments; maintain master meeting schedule
  • Produce and distribute correspondence memos, letters, faxes and forms
  • Assist in the preparation of regularly scheduled reports
  • Provide general support to visitors
  • Process incoming mail; sort and deliver accordingly
  • Order and maintain office supplies as needed for all locations; review and approve medical supply orders for all sites
  • Manage policy update process and ensure timely review and completion
  • Maintain confidentiality of all employee information
  • Perform other duties as assigned

Clinical rn - FLOAT
Clinical RN - Float (Over-the-Rhine, Western Hills and Harrison)
Reports to Clinical Operations Director

Summary: 
The clinical nurse provides a primary point of contact between patients and Crossroad Health Center. 

Clinical Duties:
  • Provide office visits to patients for specific issues, including vital sign checks, lab only visits, and medication review
  • Provide individual patient education on topics such as diabetes, insulin management, and obesity
  • Manage efficient daily clinic flow
  • Resolve patient problems utilizing a team based approach
  • Appropriately triage walk in patients
  • Manage patient phone calls in an efficient, courteous, and appropriate manner
  • Assist providers with efficient response to refill inquiries from patients and pharmacies
 
Other Duties:
  • Communicate regularly with the Clinical Operations Director and Chief Operating Officer regarding clinic policies and procedures
  • Assist the Clinical Operations Director with initial and ongoing staff education
  • Assist the Clinical Operations Director with site specific hiring of new staff and provide candidate feedback
  • Provide oversight and maintenance of the clinic emergency equipment and ensure adequate training of staff on the use of the equipment
  • Provide oversight and assist with the process of medication ordering
  • Complete other duties as assigned
 
Knowledge, Experience and Skills:
  • Must maintain an unencumbered Ohio Registered Nurse License or LPN Certification
  • Able and willing to assist with training of volunteers that may be needed
  • Good organizational skills; able to coordinate multiple tasks well
  • Good communication skills, verbal and written
  • Willingness to live and promote Crossroad’s mission
MEDICAL ASSISTANT
Responsibilities:
  • Maintain the readiness of patient care areas prior to and during office hours
  • Review day’s schedule and review/prepare records; obtain patient information needed for appointments that day
  • Obtain growth measurements, vital signs, chief complaint and other patient data according to protocol and as indicated by presenting symptoms
  • Complete procedures safely that are ordered by the provider that fall within training (including injections, EKG and venipunctures); Prepare for and assist physician with exam and treatment procedures as needed
  • Prepare specimens/requisitions for lab transport; perform in-house labs according to protocols
  • Review lab results, transcribe phone messages, call in prescriptions and contact patients with provider instructions
  • Collaborate with patient care team, promptly communicating any information that is important for continuity of care
  • Perform front desk duties as necessary: answering phones, scheduling patient appointments, printing medical records, filing, sending/receiving faxes, registering and check-out procedures
  • Attend staff meetings and participate in evaluations of performance and job satisfaction
  • Communicate feedback and concerns to supervisor in a timely and professional manner; maintain an open line of communication with providers, supervisors, and peers
  • Promote efficient, organized and pleasant office operations in the health center, consistent with the mission of CHC to offer care in an atmosphere of dignity and respect
  • Comply with HIPAA and safety procedures

  • Render care to Crossroad Health Center (CHC) patients in accordance with quality and standards as are established by CHC, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age or ability to pay
  • Provide comprehensive primary care services to children
  • Prescribe or administer treatment, therapy, medication, vaccination and other specialized medical care to treat or prevent illness, disease or injury
  • Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients' condition
  • Collect, record and maintain patient information, such as medical history, reports and examination results
  • Monitor patients' conditions and progress and reevaluate treatments as necessary
  • Explain procedures and discuss test results or prescribed treatments with patients and family to ensure understanding
  • Advise patients and family members concerning diet, activity, hygiene and disease prevention.
  • Refer patients to medical specialists or other practitioners when necessary
  • Refer patients to other health care practitioners or health resources
  • Collaborate with health care professionals to plan or provide treatment
  • Train residents, medical students, and other health care professionals
  • Provide on-call services after clinic hours for urgent pediatric patient phone calls (phone coverage only)
  • Responsible for accurate, timely and complete documentation in patient records in accordance with CHC policy
 
Qualifications:
  • Excellent written and verbal communication skills
  • Possesses a high level of personal accountability, ownership and a rigorous work ethic
  • Ability to work independently with little to no supervision
  • Strong time management skills
  • Ability to function effectively as a team member and facilitate completion of tasks.
  • Experience in FQHC (Federally Qualified Health Center) industry preferred
  • Requires strong organizational, problem-solving and analytical skills
  • Consistently employs sound judgment and discretion in managing patient and employee issues
  • Graduation from an accredited medical school with a degree of Doctor of Pediatrics; must be board eligible/certified
  • Must possess current, valid unrestricted license to practice medicine in the State of Ohio
  • Must have current, valid unrestricted DEA registration
