Payment & Insurance
We accept select Medicaid, Medicare, and private commercial and Affordable Care Act insurance plans. If you do not have insurance, our Outreach & Enrollment Specialists will help you sign up for Medicaid or a subsidized plan. Crossroad provides a sliding fee scale with a minimum co-pay of $15 for patients who do not qualify for one of these plans. While we are committed to providing affordable care for all our patients, payment is still expected at the time of service, by cash, check or Visa/Mastercard/Discover. Not having payment at the time of service may result in your appointment being rescheduled.
