Why am I getting text messages from 513-381-2247?
We are working with Inxite Health Systems to send out appointment reminders to our patients via text. If you do not wish to receive your appointment reminder via text, reply "Out" to any texts received from us.

How do I navigate the phone system?
Listen carefully to the prompts. 
There are several areas of the office that can be reached through the phone system. Dial your Crossroad Health Center locations number and listen carefully to the message at each prompt for required information. Please speak slowly, clearly and spell all names. Someone from each department will return your call, if you need to leave a message.

Who do I contact after office hours?
Even when the clinic is closed, you may call your Crossroad Health Center location for urgent medical needs. In fact, we want you to call us! There is always a provider on call for after-hour calls. Telephone calls to the doctor or nurse practitioner after our office is closed should be reserved for urgent issues regarding your immediate condition. Calls of a routine nature should be saved until the office opens in the morning. You will be directed to our after hours line. Be prepared to leave your phone number and a brief message. A provider will call you back promptly to address your health care concerns and questions. If you are having a medical emergency, please call 911.

What do I do in case of an emergency?
Call 911. Discuss what might be considered a medical emergency for you with your provider at your next appointment.

How do I have previous records sent to your office?
Complete a medical records request to have previous records sent to our office. Download and print a copy here. There may be fees associated with release of records. Please call your previous provider's office with questions about records release or fees.  

How do I have existing records sent to a new provider?
Have your new provider send us a medical records request with your signature. There may be fees associated with release of records. Please call the medical records manager at our office with questions about records release or fees.
