Your first visit

During your fist visit you will complete various forms, meet with your care team, and establish a Primary Care Provider (PCP). If you cannot make your appointment for any reason, please call us in advance to cancel the appointment so we can make that time available for someone else who needs care.

What to bring

  • Photo identification
  • Insurance card, if you have insurance
  • If you don’t have insurance, please bring proof of your household income (a recent check stub or letter from your employer), and cash or check to pay for your visit

Payment & Insurance

We accept select Medicaid, Medicare, and private commercial and Affordable Care Act insurance plans.  If you do not have insurance, our Certified Application Counselors will help you sign up for Medicaid or a subsidized plan if you are eligible. Crossroad provides a sliding fee scale with a minimum co-pay of $15 for patients who do not qualify for one of these plans. For more information, contact Crossroad's Certified Application Counselors at 513-381-2247 or email insurance@crossrd.org  

While we are committed to providing affordable care for all our patients, payment is still expected at the time of service, by cash, check or Visa/Mastercard/Discover. Not having payment at the time of service may result in your appointment being rescheduled.
