About Crossroad

Crossroad Health Center is a health clinic providing affordable, high-quality care to the children and adults of Greater Cincinnati and Harrison. No one is turned away due to inability to pay. Visit our About Us page to learn more about our mission and history.

Our services

Crossroad offers primary care services for patients of all ages. We strive to treat the whole person – body, mind and spirit. Click here to see a full list of our health services. We also help patients overcome barriers to good health, such as food insecurity or lack of permanent housing, by connecting them to medical specialists, support programs and social service agencies.

A medical home

Crossroad strives to create a trusted medical home for our patients. We have earned the designation of Patient Centered Medical Home (PCMH) from the National Committee for Quality Assurance (NCQA). As a PCMH, we put patients at the center of their care team. Our medical staff takes the time to get to know our patients so we can build relationships that lead to better care and better health outcomes. PCMH services provided by Crossroad include same-day and Saturday appointments, preventive care, school physicals and an online patient portal.

Becoming a patient

To become a Crossroad patient, call your preferred location to set up your first appointment.

Your first visit

During your fist visit you will complete various forms and meet the doctor or nurse practitioner who will be your primary care provider (PCP). If you cannot make your appointment for any reason, please call us at least three hours in advance to cancel it so we can make that time available for someone else who needs care.

What to bring:

  • Photo identification, such as a driver’s license
  • Insurance card, if you have insurance
  • If you don’t have insurance, please bring proof of your household income (a recent check stub or letter from your employer) and cash or a check to pay for your visit

Payment & Health Insurance

We accept select Medicaid, Medicare, and private commercial and Affordable Care Act (Obamacare) insurance plans.  See a complete list of accepted plans here. If you do not have insurance, ask to talk with one Crossroad's Certified Application Counselors. They can help you sign up for Medicaid, Medicare or a subsidized plan if you are eligible. Crossroad provides a sliding fee scale (link to Sliding Fee chart) based on your income. There is a minimum co-pay of $15 if you do not qualify for a health insurance plan.

We are committed to providing affordable care for all our patients. However, payment is still expected at the time of service, by cash, check or Visa/Mastercard/Discover. If you cannot pay at the time of service, your appointment may need to be rescheduled.
