Advanced Practice Provider Residency Program

We Welcome Nurse Practitioners and Physician Assistants!

Crossroad is pleased to offer an Advanced Practice Provider Residency Program for newly graduated nurse practitioners and physician assistants. Our APP Residency Program, launched in 2024, leverages our diverse, experienced Christian provider workforce to mentor the rising generation of nurse practitioners and physician assistants who are committed to caring for the underserved. We are a Christian faith-based Federally Qualified Health Center that partners primarily with our local and regional training institutions. We welcome nurse practitioners and physician assistants who align with our mission: to honor Jesus Christ by providing accessible, comprehensive health care in partnership with our patients and community leading to healthy, changed lives.

 Mission

In support of our faith-based mission, the APP Residency Program’s mission is to build competent and confident advanced practice providers equipped to provide wholistic, high-quality care to underserved populations.

Vision

Our vision is to develop advanced practice providers who are equipped to care for the complex patients seen in FQHCs. After completing residency, our advanced practice providers will be prepared to provide excellent care to underserved populations, helping to increase access to affordable, accessible care.

Goals

  • To train and equip newly graduated APPs with the knowledge, skills and attitude needed to care for complex and underserved patient populations.
  • To mentor newly graduated APPs to become well-trained providers in the primary care workforce.
  • To develop APPs who can integrate ethical, whole-person care, including spiritual care and prayer, into their primary care.
  • To prepare future primary care providers to join the Crossroad Health Center team.

Program Description

We offer a one-year program including didactics and precepting with a guaranteed full-time offer to work at one of our Crossroad locations following residency after successful completion of the program.

Resident Qualifications

We are seeking family nurse practitioners or physician assistants who have completed an accredited program in the last 12 months.

Application

Applications accepted: January 1-February 28, 2025

Interviews: March and April 2025

Final decision: May 1, 2025

Deadline to accept position: June 1, 2025

Start date: July 15, 2025
